The right-most car with the pantograph (behind the driving car) turns around
So that's the thing: it's actually not turning around, only the two pantographs are switching (one goes up the other goes down); if the whole car turned around the door would be at the wrong position. Effectively what it means is that the pantograph first in direction of travel is always raised (even though as discussed with robert357 this is not common), but the consist stays in the same order, simply the direction reverses. Compared to what happens in OTTD 15, the whole train would swap direction which means also reverse the order of the consist
