It's a known problem if you use old extensions based on the addon SDK (these can be identified by not requiring a restart after install, and not having any separate preferences/options dialog; there's just a preferences section for them in the addons manager. Something to do with how they dynamically add their UI elements instead of being coded in static XUL. I already use Cookie Masters and it caused no problems; Cookie Control panel on the other hand instantly added a button without prompting for a restart. So disable it and test your toolbar customizing again.I'd say there's no need for it if you already have Cookie Masters.
