OK, Moonchild, I saw your post after I answered athenian200. That seems to have solved the problem; maybe I accidentally clicked on that icon at some point. But I still have that icon I mentioned in the EDIT of my original post.

That is the history button. Just "customize" your toolbars and remove it if you don't want it.
