Who Should We Blame for Medical Complications?

When the worst thing imaginable happens during a medical procedure or intervention, what happens next? I decided to make this video after learning about a tragic case of a young boy who lost his life. Many mistakes were made, based on the coroner's report, but the learning points and the focus of media coverage seems to be at odds with my interpretation of the case. So I wanted to explain a bit about how we approach complications.
