How can I communicate hazard ratios from log-transformed biomarkers in a clinically interpretable way?

I wouldn’t trust log transformations, instead opting for restricted cubic spline functions of log values. To describe the results make partial effect plots and compute inter-quartile-range effect ratios as described here.
