A character who died by beheading in a previous campaign returns on a new adventure, except now his head is grafted onto a chicken’s body. He’s adjusted quite well to writing and holding items with his feet, finds it liberating to not have to wear clothes anymore, and the only downside, from his perspective at least, is that he now occasionally lays an egg.

This seems sufficiently specific that I start to suspect that this is not an idea but propaganda for the "grafting your head onto a chicken" lobby.
The worst part is, it's working.
