A hydrogen internal combustion engine is provided with at least one cylinder (having an inlet to feed oxidizing air into the cylinder and with an outlet to let combustion gases out of the cylinder, at least one piston engaged in a sliding manner in the cylinder, and an injection unit to feed fuel hydrogen into the cylinder; the injection unit having a feeding manifold, which extends around at least part of a side wall of the cylinder, and communicates with the cylinder through a plurality of feeding holes obtained through the side wall of the cylinder.
