A drum brake apparatus (11) includes a wheel cylinder (15), an extension mechanism (21) disposed in the wheel cylinder, and an electric actuator. The wheel cylinder (15) pushes adjacent ends on one sides of a pair of brake shoes (25, 27) disposed in a brake drum (29), at a tip portion of a first piston (39) and a tip portion of a second piston (41) by fluid pressure, to thereby expands the pair of brake shoes (25, 27). The extension mechanism (21) drives the first piston (39) and the second piston (41) in a direction away from each other. The electric actuator rotates the extension mechanism (21).
