The present invention relates to a modified conjugated diene-based polymer and a rubber composition including the same, and provides a modified conjugated diene-based polymer satisfying the conditions of i) a mooney viscosity measured in ASTM D1646 conditions and at 110°C: from 75 to less than 100, ii) a mooney stress relaxation ratio measured at 110°C: 0.4 or less, iii) a weight average molecular weight: 750,000 g/mol or more, and iv) molecular weight distribution (PDI; MWD): from 1.0 to 2.0.
