Codex tasks succeed but PRs fail

Very often a situation occurs where codex has completed the task, I see the summary in the log, codex has described in detail what was done, but the task preloader keeps spinning and the task does not move to the completed status and, accordingly, there is no option to create a PR. Has anyone encountered this and what is the solution?
