MCP Apps: ui/notifications/tool-result missing _meta in ChatGPT

Cross-posting here: MCP Apps in ChatGPT are fundamentally broken - 2 critical bugs ChatGPT Apps SDK > Hey all, sorry this took so long–to be frank, it took me a bit of time personally to understand why this was an issue (I spend more time on the top-level MCP spec rather than the apps extension).
