Facing Instability in GPT-4o-Mini and GPT-4.1-Mini Models

Overall response quality of GPT-4o-Mini and GPT-4.1-mini has degraded .. the same prompts now return incorrect answers, especially for numerical queries (from last 2-3 weeks)
