Since you changed the setting "From visited", try setting it back to "Always" for awhile and see if you encounter the forced logged out problem again.

I've set it back to "Always" and after I redid Moonchild's steps I encountered a forced logged out again... Definitely not that one it seems. I guess it's either my random theory or my browser profile having gremlins...
