I have never done this when I had an iphone before, I know that:
> Traffic to Apple services can bypass the VPN tunnel. This includes iCloud, Maps, Siri and notifications
Mullvad VPN iOS limitations
But on a router level, I don’t think I’ve read anything to show that Apple traffic can or can not bypass it.
However I don’t think Apple will be easily fooled by this.
As looking at the information they have for the EU side loading:
> Only users based in the European Union or Japan are able to install apps through alternative app distribution. The country or region of your Apple Account must be set to one of those countries or regions, and you must physically be located there.

So as well as an account to set to that region, which I imagine Apple will want a billing address for. They will want to know your physical located there. I think there are ways to spoof iOS location, but I doubt any of them are open source and privacy respecting (although been awhile since I last looked into it).
If you manage to have any success with it, I would be interested in seeing the results and what you found.
But I think for all of these hurdles Apple is having to put in place, I’d just go and get GrapheneOS
